Transition Toolkit

The Transition Toolkit provides tools for educators helping students with TBI to become more self aware, to set reasonable and achievable goals, and to improve transition outcomes. The tools support the five practices from Kohler’s Taxonomy for Transition Programming: student development, student-focused planning, family involvement, program structure, and agency collaboration. The core tools and specialized planning tools include examples and directions for use specifically for students with TBI. The toolkit, including blank tool forms, is provided in PDF format.
